Forms & Surveys App lets you build and display sophisticated forms and surveys to gather information. which can be used to generate Jive content or be downloaded. Forms & Surveys app is a part of Jive Solutions offering.

Information

Installing the Forms & Surveys App requires Professional Services engagement. Please note that Professional Services engagements may incur additional charges. You can get in touch with your Account Manager/Executive for more details.

In order to facilitate the installation, please contact us if not done already. If you already have the app installed, you can refer to the Forms & Surveys App: User Guide on configuring the app.

If after installation, you see the error Not Authorized to Access Forms Admin Area while trying to access the Forms & Surveys app, please ensure that the affected user account is added to the relevant user groups as listed below:

• forms-surveys-admins (members can create Forms and Surveys and manage the app configuration)
• forms-surveys-authors (members can create Forms and Surveys)
